Care360 Focuses on Triple Aim

One of the hottest topics in the healthcare industry today is the Triple Aim model for healthcare reform. Triple Aim is a framework developed by the Institute for Healthcare Improvement (IHI) that focuses on three critical objectives that together, can lead to better healthcare models. The Triple Aims are:

  • Improve the health of the population
  • Enhance the patient care experience
  • Reduce the per capita cost of care

Care360 is part of a network from Quest Diagnostics that includes more than 200,000 physicians in 80,000 physician offices in the U.S. These providers can send and receive information securely and share patient charts between practices. Providers using other Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems can still receive information from Care360 EHR via secure Direct Messaging or fax. In addition, Care360 facilitates connections to pharmacies, laboratories and even health insurance companies.

EHRs Can Help Improve Patient Outcomes

Many studies highlight the improved patient outcomes experienced by users of electronic health records, such as a study published in the New England Journal of Medicine (NEJM). The study found that the use of EHRs improved the quality of care for diabetes patients. Tools like clinical decision support and informatics assist with identifying patients who may be due for follow-up care. Automatic reminders for standing lab orders help to keep patients on track.

Quickly Identify Concerns

Alerts related to drug-to-drug interaction or out-of-range lab results are an essential function of the Care360 platform. These alerts simultaneously meet the three aspects of the Triple Aim initiatives by ensuring that patients have the best quality care outcomes through prevention of adverse drug events, as well as alerts of potential health issues. Care360 also offers tiered drug formulary recommendations, leading to reduced healthcare costs, and can help manage the cost of healthcare through error prevention.

Care360 EHR is cloud-based and offers a modular approach to adoption so community practices can transition from paper to electronic management in a stepwise approach.   1. The new changes are great but I do not like the format of the note. I like to separate the medical problems, physical exam, plan into bullets. The current format puts it all into 1 paragraph that runs together. This is difficult to review and proof read because it’s not separated. I only initially saw this note format with using Google. The format was different/desired with Internet explorer. Is there a way to fix this?? Thank you, Brenda Thomas, PAC
